Duodenal switch surgery is a very complex obesity-metabolic surgery, also known as duodenal switch with biliopancreatic diversion. In the light of scientific knowledge, it is the method in which the best results are obtained in the treatment of diabetes and these effects last for the longest time. Despite these positive effects, it causes vitamin, mineral and protein deficiency in the long run. SADI-S, developed for this reason, is a more current and safer modification of this highly effective surgery.
This method, also called loop duodenal switch surgery, is a highly effective surgery with its restrictive and serious absorption-reducing effects, both in weight loss and in solving metabolic problems, especially diabetes.
How is Loop Duodenal Switch Surgery Performed?
The first stage of the surgery is to remove 75-80% of the stomach, as in sleeve gastrectomy surgery. This part creates the effect of restricting food intake and also reduces the secretion of Ghrelin (hunger hormone). In the second stage, the duodenum is cut a few centimeters ahead of the ring-shaped muscle layer called pylorus at the exit of the stomach and slightly behind the place where bile-pancreatic fluids pour out. The stomach outlet is connected to the small intestine at a distance of 250-300 cm backwards from the small intestine-large intestine junction. That is, the length of the intestine where bile and other digestive enzymes mix with food and cause absorption is reduced to 250 - 300 cm. Thus, it reduces the amount of absorption of fats and calories in foods. At the same time, direct contact of food with a lower part of the small intestine significantly reduces insulin resistance in muscle tissue, allowing for long-term permanent Type 2 diabetes treatment.
Leaving sufficient small intestine space in this surgery minimizes vitamin-mineral malabsorption, while its weight loss effect is quite high.
If you are considering this surgery, especially for a reason such as uncontrolled diabetes, you should know that you must follow a strict follow-up program and use vitamin-mineral supplements for the rest of your life. However, provided that this condition is met, we can say that SADI-S surgery is extremely effective and safe.
The most obvious side effect of this surgery is frequent and foul-smelling diarrhea in the early period after surgery. After the intestinal adaptation process, this complaint gradually decreases.
